1. Patient’s Information
Patients information section consists of the following information:
- Name –The patient’s full name.
- DOB – The patient’s date of birth.
- Address – The patient’s residential address.
- SSN – The patient’s Social Security Number.
- Doctor – The name of the attending physician.
- Doctor’s Contact # – The contact number of the attending physician.
- Pharmacy – The name of the pharmacy where medications are procured.
- Pharmacy Contact # – The contact number of the pharmacy.
2. Daily Log
The daily log consists of the following columns:
Medication: The name of the prescribed medication.
Dosage: The recommended dosage of the medication.
Units: The unit of measurement for the medication, such as tablets, milliliters, injections, or DNS (Did Not Specify).
Date: The date on which the medication was taken.
Time: The time at which the medication was administered.
Remarks: This section allows for the recording of pertinent information, such as the reason for taking the medication, its observed effects, and any post-medication vital sign measurements, including blood sugar or blood pressure readings.
